Published in 1917 by George Banta Publishing Company, Winconsin. This pamphlet describes a number of US forms, as in alphabetic order of the title of that form. It also lists reports and returns as to when to do which one. A further part deals with forms and stationary to be kept in Company Office, incl. Supplies to be carried in Field Desk (as per Q.M.G.O. Cir. of August 17, 1916). All in all a very informative booklet when doin US WWI re-enactment.
